### Required Information
**General Information:**
– Your full name
– Address (City, State, Zip Code)
– Phone number
– Alternate phone number (if available)
**Obituary Specification:**
– Name of the deceased
– Complete obituary text
– A photo attachment is preferred in JPEG or PDF format. TIF and other file types are also accepted; we will contact you if there are any issues with the photo.
– Requested ad run dates
*(Discounts may be available for multiple-day placements. For the discount to apply, scheduling must occur on the first run date and the same photo must be used for all days. Contact us for details.)*

### Important Policies
**Verification of Death:**
To publish an obituary, we require the name and phone number of the funeral home or cremation society managing arrangements. We must contact them during their business hours to verify the death.
– If the deceased’s remains were donated to the University of Minnesota Anatomy Bequest Program (or similar), provide their phone number for verification.
– Alternatively, a death certificate is acceptable (only one form of verification is needed).
– Please allow sufficient time for verification, especially on weekends due to limited business hours.
**Guestbook and External Websites:**
We cannot reference other media sources, guestbooks, or obituaries from outside sources (e.g., other newspapers or memorial websites).
– We can, however, include a website for the funeral home or a family email address for contact.

### Obituary Process Overview
1. **Proof Sent for Approval:**
After your submission is received, we will send you a proof by fax or email with pricing and run dates for your review.
2. **Review and Corrections:**
Please verify all details. Notify us of any changes or errors before final publication deadlines. Pioneer Press is not responsible for errors identified after publication.
3. **Online Obituary Changes:**
Adjustments to published online obituaries can be coordinated through the obituary desk. Call with any questions.

### Payment Information
– **Pre-payment is required** for all obituary notices before publication.
– Once you have received and approved your proof, call **651-228-5263** to make your payment.
– Accepted payment methods:
– **Credit Card:** By phone only (for security reasons).
– **EFT (Check by phone):** Provide your routing and account numbers.

### Rates
– **Minimum charge:** $162 for up to 12 lines.
– **Each additional line:** $12 per line after the first 12 lines.
– **Obituaries over 40 lines:** Receive a 7.5% discount per line.
– **Multiple Days:** Schedule your second run date for a 20% discount on both placements. Place three obituaries and the third will be free.
– **Photo Charge:** $125 per photo per day (e.g., two days with one photo = $250; two photos for two days = $500).
